If you are looking for massage therapy following an auto accident and would like to bill a motor vehicle insurance claim, prior to your first appointment, please email me the following:

1)  Prescription from Doctor or Chiropractic Physician

A prescription for massage therapy must include the following information:

  • The referring physician's full contact information, including their National Provider Identification (NPI) number

  • The diagnosis code(s)

  • The number of sessions prescribed

  • CPT codes (97124, 97140) that indicate massage and manual therapy

2) MVA Intake Form

  • Download and email me a completed copy of the MVA Intake form

  • Insurance company’s name, address and phone number

  • Claim number and the date of incident

  • Claim adjuster's name, telephone number and email address
